I am writing to inform you of an important update regarding our practice. After much consideration, I have made the difficult decision to close my office in 236 Stockbridge Rd STE B-2, Jonesboro, Georgia 30236 effective April 1, 2025. This decision was not made lightly, and I deeply appreciate the trust and support you have shown me.

Beginning in April, I will be working full-time  at YourTown Health QuickCare in Palmetto, GA. I am excited about this new opportunity, but I understand it may cause some inconvenience for you. To ensure continuity of care, I have listed local primary care providers that are accepting new patients. You may also contact your insurance company for additional recommendations.

If you wish to transfer your medical records to another provider, please complete and return the form at the link below at your earliest convenience. This will allow me to send your records promptly and ensure your healthcare needs continue to be met without interruption.
